The
landscape of Value Based Care presents SNF leaders with clinical and
financial challenges while ensuring best care outcomes for our

planning, building the Mission team, overcoming obstacles, measuring
Mission outcomes to conclude the Mission with clinical and financial
excellence.
This
fun-filled session takes place in a Mission Impossible theme.
Attendees, should they choose to accept the Mission, will take part in a
high-stakes session to understand their role and the role of their
Mission Team in successfully navigating the challenge-filled sector of
value-based care! Failure to complete this mission could result in poor
resident outcomes, loss of QIP dollars, decreased referrals and an
unskilled Mission Team.
What You Will Learn
- Setting the Stage for Mission Possible - Upon completion
participants will identify what's at stake in VBC, know how to build the
best team and the role of each member.
- Assessing the Mission and Adapting the Plan - Upon completion
participants will define appropriate outcome measures, understand how to
course correct strategically, and establish effective and efficient
feedback loops.
- Overcoming Obstacles - Upon completion participants will be able to
address common Mission obstacles such as staffing, patient complexity,
and the pressures of value-based metrics.
- Mission Accomplished - Upon completion participants will understand
how to achieve clinical excellence and financial success under VBC.
